Establishing the Rural Cancer Institute: Advancing Advocacy and Research in Rural Cancer Care

Abstract
Background
Rural communities face unique challenges in cancer care delivery. Yet, no dedicated national platform addresses these disparities. Recognizing this gap, the Rural Cancer Institute (RCI) founders convened at the 2022 National Rural Health Association Annual Meeting. Their shared vision was to improve cancer health equity in rural areas through research, advocacy, and policy initiatives.
Case Discussion
RCI has undertaken multiple efforts to advance rural cancer care since its inception. Key initiatives include: Research Collaboration: Monthly research meetings engage medical students from five medical schools, fostering interdisciplinary collaboration and mentorship. Scientific Contributions: RCI members have presented multiple posters and oral presentations at national conferences, raising awareness of rural cancer disparities. Publications: Scholarly work has been published in high-impact journals, including NEJM Catalyst and JAMA Oncology, to disseminate findings on rural cancer care challenges and solutions. Legislative Advocacy: Advocacy efforts have extended to the Minnesota Capitol and the Kansas State Senate, influencing policy discussions on rural oncology care. Public Awareness: RCI has actively engaged in media advocacy through letters to the editor in the Wall Street Journal and the Minnesota Star Tribune. State-Level Recognition: Efforts have led to gubernatorial proclamations in six states declaring June 4 as Rural Cancer Awareness Day, enhancing public awareness. Research Agenda Development: Worked with the Patient-Centered Outcomes Research Institute (PCORI) to help shape a research agenda focused on addressing rural cancer disparities. National Convening: RCI will host its inaugural national meeting in Lawrence, Kansas, on June 6-7, 2025, bringing together stakeholders to further rural cancer health equity initiatives.
Conclusion
The Rural Cancer Institute has emerged as a force in addressing rural cancer disparities through research, advocacy, and policy engagement. By fostering collaboration and elevating rural oncology issues on national and state platforms, RCI continues to advance the mission of equitable cancer care for rural populations.
